Email vaccination history - Transfer veterinary records

Please email or text a photo of your pet's most recent vaccinations at least 24 hrs prior to your appointment. If you prefer, I can call your veterinarian to get the records, but most vets require you to call ahead to release pet records. Please provide me with the name and telephone number of your veterinarian if you would like for me to get your pet's records. Please note that if your pet has a chronic condition, I need to review the records prior to the first appointment, so please email records for review to

Gather any current medications, including flea/tick/heartworm prevention products

I will want to make notes of current medications in your chart and check dosages during our first appointment. 

Look over our packages & insurance information

We will be glad to answer any questions you have regarding promotions, packages, insurance, etc. at your appointment. We want to be a partner with you in your pet's health care. We will find the most affordable and practical options for you and your four legged family members. By reviewing options before the appointment, you may be better prepared during the actual appointment and we can spend more time discussing your pet's health instead of discussing details of costs and service plans.

Make a list of questions/concerns for the appointment

I often address many common questions/concerns during an appointment, but please have your questions ready so that you don't forget them. I'll be happy to answer them or make recommendations for diagnostics/treatments if additional information is needed.

Forget something during the appointment?

Feel free to email or text me after the appointment if you think of additional questions. If you need a more lengthy phone consult, additional fees may apply as I will need to block off my appointment schedule to accommodate those types of requests. If your pet has samples taken for diagnostics, I (or my assistant) will follow up with the results in the next 2-7 business days (turnaround time varies with the specific test being performed).
